*Note:  Submitting this application is 1 of 3 required forms.  The 2 other required forms must be filled out in person, and can be done at Volunteer Orientation.  Your application will not be fully complete until we receive this application, the Consent and Release for Criminal Background Check (filled out at Volunteer Orientation), and Policy and Procedure Acknowledgement (filled out at Volunteer orientation) in our office.  Please don't hesitate to contact us if you have any questions.
